All NEW permits for Body Art Establishment, and Nail Salons applications must be submitted by mail using a paper form and check/money order permit fee payment.

Once you create an account or log in to your existing account you will be able to submit reports of potential environmental health hazards or violations of the city’s tobacco control regulations as well as apply for permits in the following fields:
