To gain access to your account and see your Health Department records you must first log in.
Please use the "Login" link on the top right to log into your account. If you have not opened an account before, please use the "Register for an Account" on the top right to create an account.
Once you are logged in, you will be able to access all other Health Department record types, such as submitting an application, paying an invoice, or filing a complaint. If you are logged in, you should be able to see your previously submitted records above and the “Create an Application” link on the left. Please use the “Create an Application” link to submit an application or complaint.
